What is TRT?

Testosterone is a hormone found in the body that helps promote male characteristics and maintain normal development.

Testosterone Replacement Therapy (TRT) is a medical treatment for low testosterone.

Approximately 25% of men aged 40-62 years experience low testosterone.

Testosterone comes in several forms, including oral capsules, topical patch and cream, and injection. They all work to increase levels of testosterone in your body.

Testosterone levels in a normal range positively affect your vitality, libidio and energy levels.

How do I know if I have low T?

Common symptoms of low testosterone include decreased libido, fatigue, erectile dysfunction and mood changes.

However, you must also complete bloodwork to determine if your testosterone levels are also low, and that there is not another primary cause for these symptoms.

What causes low T?

There can be several other medical conditions that can decrease your testosterone or are associated with low testosterone such as obesity, sleep apnea, type 2 diabetes, metabolic syndrome, osteoporosis, HIV/AIDs and some medications.

Improving testosterone levels can help improve body composition in these conditions, notably, metabolic syndrome and obesity with a combination of dietary changes and exercise.

Why are other medications used in addition to TRT?

Occasionally, other medications can be prescribed to keep hormones that have a role in testosterone levels within the normal range.

Anastrozole: Anastrozole is a non steroidal aromatase inhibitor. Occasionally when taking testosterone replacement therapy some testosterone can aromatize which increases estradiol (estrogen) levels. Anastrozole blocks this process from occurring. It can help with side effects such as mood changes, and gynecomastia (increased breast tissue).

Clomiphene: Clomiphene can help preserve some fertility as well as improve testosterone levels modestly. It can help improve symptoms such as fatigue, libidio and mood.

How to use Anastrozole
Here’s how to use the medication

Anastrozole is in a class of medication called aromatase inhibitors and can be used to decrease estradiol levels.

Dose: 0.5mg capsule once or twice a week. It can be taken with or without food.

Missed dose: Take next dose as soon as you remember, unless it is your next dosage day, then take as normal.

Common side effects:

  • hypertension
  • edema
  • joint pain, general pain
  • nausea
  • fatigue
  • mood changes
  • insomnia (with long term treatment or high doses – not doses typically prescribed)
  • bone loss
  • decreased libido
  • fatigue
  • irritability

 

Contraindications:

  • Hypersensitivity or Allergy to Clomiphene Citrate
  • Liver Disease / Dysfunction
  • Uncontrolled thyroid or adrenal dysfunction
  • Presence of an organic intracranial lesion such as a pituitary tumour
  • Known or suspected testicular tumours or breast cancer.
How to use Clomiphene
Here’s how to use the medication

Clomiphene is a non hormonal method to increase testosterone. it can improve fatigue libido and decreased erections

Dosage: Take one capsule daily. It can be taken with or without food.

Missed dose: Do not double up dose, take next dose at regular time.

Side effects: Common side effects may include

  • flushing
  • bloating or abdominal pain,
  • nausea,
  • dizziness
  • fatigue
  • headache

Contraindications:

  • Hypersensitivity
  • Osteoporosis
  • Ischemic heart disease
  • Liver disease
Storage, Stability, Disposal

Store the medication at room temperature in a cool dry place (bathrooms are a no-go). The medication will expire after 6 months. If you need to dispose of any extra medication, please return the capsules to your local pharmacy.

Pharmacy FAQs

How do I get my prescription?

Once your prescription is ready you can either pick it up at our on site location (Unit 400-2620 Bristol Cir, Oakville, ON L6H 6Z7) or have your medication delivered monthly to your home for free. We ship across Canada to all regions except for: New Brunswick, Northwest Territories, Prince Edward Island, Nunavut, and Yukon. Once your order has been processed, you’ll be provided with a tracking number and can expect your delivery within 3-5 days. You will need to sign for your package, or pick it up with valid ID from Canada Post.

How do I get a refill?

You will be auto-refilled every 1-3 months (depending on your original prescription) as long as you remain on the program and keep up with your bloodwork.

How do we connect if there’s any problems?

How does follow up work?

During month 1-3:

  • We start regular check-ins and content to track and support your progress.
  • You will receive several emails from us about how to build healthy habits – written by our medical team.
  • You will chat with our medical team every 3 months to check your symptoms, and review your bloodwork.


Every 3 months thereafter:

  • Every three months you’ll get new bloodwork done so we can review and make sure you’re progressing well and no issues are coming up.
  • A nurse practitioner follow up and review will be every 3-5 months.
General FAQs

Can I drink alcohol while on this medication?

In general, drinking alcohol excessively can have short and long term effects to hormone balance, including testosterone – the primary male sex hormone. Alcohol can disrupt testosterone productions by interfering with the glands that secrete testosterone. Heavy drinkers (i.e. more than 15 drinks/week) tend to have lower testosterone levels than those who consume a moderate amount of alcohol.

Thus, in general, it is recommended to minimize the consumption of alcohol while on testosterone replacement therapy – in order to increase the efficacy of therapy.

What if I get sick with a flu or flu like symptoms while on treatment?

While rare, it’s possible to experience flu like symptoms while on testosterone replacement therapy. These side effects should be temporary and manageable with rest, adequate fluid intake and over the counter medications like Advil and Tylenol for pain management. Please reach out to our team if you are having prolonged flu like side effects or severe issues while on treatment.

Can I switch medications if it doesn’t work?

There are different formulations of testosterone available as replacement therapy, including patches, injections, gels and capsules. However, all have a side effect profile that may be unfavourable to many patients. If you’re experiencing side effects related to the dosage form chosen for you, please speak to our health care providers to discuss your concerns.

In terms of knowing whether or not therapy is working for you, effects on sexual interest appear after 3 weeks plateauing at 6 weeks, with no further increments expected beyond. Changes in erections/ejaculations may require up to 6 months. Effects on quality of life manifest within 3–4 weeks, but maximum benefits take longer.
