After Payment:
Once your payment is confirmed, log into your Jack Health account to get started
Find Your Order:
In your account, click on the recent order. At the top, you'll see a "Consult Questionnaire" ready for you to fill out.
Complete the Consult:
Complete the initial consult questionnaire to help us route you to the registered wellness professional in your area.
Book Your Time:
After completing the questionnaire, you’ll be directed to a booking page where you can choose the best time to meet with the registered wellness professional.
